Medicare can be expensive, but you don’t have to manage the costs alone. The Medicare Savings Program helps eligible individuals pay for Medicare expenses, including premiums, deductibles, copayments, coinsurance, and prescription drug costs.

If you qualify, this program can help you save money and reduce your out-of-pocket healthcare expenses—giving you peace of mind and financial relief.

Eating well shouldn’t be a struggle. The Elderly Simplified Application Project (ESAP) makes it easier for older adults to apply for Supplemental Nutrition Assistance Program (SNAP) benefits. SNAP helps you stretch your grocery budget so you can buy food at grocery stores, farmers’ markets, and other approved locations.

You may qualify if:

• You are 60 or older
• You have no earned income
• You are not already receiving SNAP benefits

More than half of Americans over 65 will need long-term care at some point in their lives. Whether it’s assistance with daily tasks like dressing and bathing, skilled nursing care, or in-home support, long-term care services help older adults maintain their well-being and independence.

Long-term care looks different for everyone. It can be provided in a nursing facility, assisted living community, or even in your own home, depending on what you need. But for many South Carolinians, the cost of long-term care can be overwhelming.

Healthy Connections Medicaid may be able to help. If you or a loved one needs long-term care, SC Thrive can guide you through the process, screen for eligibility, and help with your application.

Caregivers are our unsung heroes. Today, one in five adults in the United States is caring for an aging loved one. That number will only grow. Caregiving is both rewarding and exhausting, but finding help and adequate resources is daunting. SC Thrive is addressing these challenges, linking caregivers with key providers and raising awareness through Caregiver Simulations and trainings.

Healthy Connections Prime

Healthy Connections Prime is an enhanced healthcare option for adults aged 65+ providing enhanced Medicare and Medicaid services. This program streamlines care and benefits by grouping your Medicare, Medicaid, and Part D services under a health plan you choose.

Through Healthy Connections Prime, you receive a care manager and team who will work directly with you and your doctors, ensuring you receive the best care possible.
