Update Your Address through SCDHHS’s New Online Portal

South Carolina Department of Health and Human Services (SCDHHS) made a change so you can update your address more effortlessly. SCDHHS just released a new portal that allows Healthy Connections Medicaid members to update their address and contact information online. The online portal enables members to update their information quickly and when it is convenient for them. 

Healthy Connections Medicaid members must have accurate and up-to-date contact information so they do not miss information about their health care coverage. This will become even more important once SCDHHS resumes its required annual eligibility reviews, which have been paused due to the pandemic.   

To update a home or mailing address online, the member will need their Medicaid identification number or Social Security number, date of birth, phone number and new address. Healthy Connections Medicaid members can also update their address and contact information by calling 888. 549.0820 Monday through Friday from 8:00 AM to 6:00 PM or visiting their local eligibility office.
