Many older adults have limited incomes, but some of their expenses increase on things like prescriptions or doctor visits. It’s important for everyone to know that there are programs available to help them save on Medicare premiums, pay for their home health care, or help put food on their tables. No older adult should have to go without the things they need, especially when there are options available to help. SC Thrive has several ways that we can help those in need. We can help older adults apply for Healthy Connections Prime, Long-Term Care benefits, the Medicare Savings Program, & SNAP’s Elderly Simplified Application Process (ESAP).

Healthy Connections Prime

Healthy Connections Prime is an enhanced healthcare option for adults aged 65+ providing enhanced Medicare and Medicaid services. Healthy Connections Prime offers better care by making it easier for you to get all of your Medicare, Medicaid and Part D services from a health plan that you choose, better value through a care team and a care manager that works directly with you and your doctors and better health through benefits that help you stay at home with your family for as long as possible. Long-Term Care

Over 50 percent of Americans 65 or older will need long-term care services in their lifetime. Long-term care includes both medical and non-medical care for people who need help to meet health and personal needs and to perform basic activities of daily living like dressing and bathing. These services are available for individuals who require a level of care like that offered in a nursing facility. Long-term care services do not have to be provided in a nursing facility. There are different types of long-term care services from those that can be provided in the home, to different levels of assisted living, to nursing home care. The type of care can be tailored to the needs and resources of the individual. In South Carolina, many older adults need long-term care services but cannot afford to pay for them. Medicare Savings Program

Want to save money on your Medicare costs? Put money back in your pocket with the Medicare Savings Program! The Medicare Savings Program helps pay for some or all out-of-pocket costs such as Medicare premiums, deductibles, copayments, coinsurance, and prescription drug costs for qualifying individuals. SNAP Elderly Simplified Application Project (ESAP)

ESAP allows you to easily apply for the Supplemental Nutrition Assistance Program (SNAP). SNAP helps you increase your grocery budget by allowing you to buy food at grocery stores, farmers’ markets, and more. You could qualify if your household members are at least 60 years old, do not have earned income and do not already receive SNAP benefits. You can learn more about SNAP benefits HERE.
